EDEN's HUD Multi-Family Waitlist Application form will be available starting
March 6, 2025, at 8:30 am. All forms must be submitted
by 4:30 pm on March 7, 2025.

Prepare to complete the form!

1) Download and print the Disability Verification form and ask your doctor to complete and sign it. Scan it or take a photo so that you can attached it to your form. (Photos taken by phone will be accepted.)

2) Get copies of a photo ID for each member of your household. (Photos of an official ID taken by phone will be accepted.)

3) Gather information about all members of your household:

• Current address or address where you can receive mail.
• Names, date of birth and social security numbers for everyone.
• If you are linked with case management, your case manager's name and contact information (phone number and/or email address)
• Annual income by sources, if any, for all household members aged 18 and over.

4) Plan to the complete the online form from March 6, 8:30 am, through March 7, 4:30 pm.

5) Please let us know if you need an accommodation to help complete this submission—such as needing an interpreter or an accommodation due to a disability, etc.—by emailing edenHUDwaitlist@EDENcle.org or calling 216-961-9690 ext. 225.
